Appointment automation: cutting no shows in a weight loss clinic

7 min read · Updated August 2026

No shows are pure margin loss. Acquisition is already paid for, provider time is already blocked, and the slot cannot be resold at short notice unless a system is watching.

This playbook covers the three levers that move show rate: reminder design, commitment at booking, and automated recovery.

Reminder cadence that people actually read

Text outperforms email for reminders by a wide margin. Three touches is the sweet spot: 72 hours out to allow rescheduling, 24 hours out to confirm, and 2 hours out with parking or telehealth link details.

Every reminder should include a one tap confirm and a one tap reschedule. Forcing a phone call to move an appointment converts a reschedule into a no show.

Create commitment at the moment of booking

A small refundable deposit, clearly explained, reliably improves show rate. So does a short confirmation message that names the provider and states what the consult covers, because the appointment becomes specific rather than abstract.

Automate recovery within the hour

When a patient misses a consult, an immediate friendly text with two named alternative slots recovers a meaningful share. Waiting until the next day usually loses them.

Cancellations should trigger the same path, and freed slots should be offered automatically to a short waitlist so provider time is not lost.

Measure by cohort, not in aggregate

Show rate varies by traffic source, day of week, lead time and provider. Reviewing it by cohort tells you whether to change reminders, scheduling windows or the offer itself. Consults booked more than ten days out almost always need an extra touch.
